Registration Forms

In order to speed up the registration process, you can complete the GMS1 Form and our New Patient Health Check Questionnaire, Registration Form and Audit C form in advance of coming to the practice by downloading them.

Please use the checklist to ensure you complete all of the required forms.

Safe Surgery


The Peninsula Practice is pleased to announce that we are a recognised Safe Surgery, a scheme run by Doctors of the World to ensure no one is excluded from the healthcare they have the right to.

By being a Safe Surgery, we commit to prevent things such as ID, proof of address and language from being barriers to patient registration.

This is one step we are taking to tackle health inequalities in our communities.

Temporary Patient Registrations

If you are ill while away from home or if you are not registered with a doctor but need to see one, you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.

Out Of Area Registration

New arrangements introduced from January 2015 give people greater choice when choosing a GP practice. Patients may approach any GP practice, even if they live outside the practice area, to see if they will be accepted on to the patient list.

GP practices have always had the ability to accept patients who live outside their practice area. Regardless of distance from the practice, the practice would still provide a home visit if clinically necessary. The new arrangements mean GP practices now have the option to register patients who live outside the practice area but without any obligation to provide home visits.

Out of area registration (with or without home visits) is voluntary for GP practices meaning patients may be refused because they live out of area.

If your application is considered the GP practice will only register you without home visits if it is clinically appropriate and practical in your individual case.

To do this we may:

  • Ask you or the practice you are currently registered with questions about your health to help decide whether to register you in this way.
  • Ask you questions about why it is practical for you to attend this practice (for example, how many days during the week you would normally be able to attend).

If accepted, you will attend the practice and receive the full range of services provided as normal at the surgery. If you have an urgent care need and the surgery cannot help you at home we may ask you to call NHS 111 and they will put you in touch with a local service (this may be a face to face appointment with a local healthcare professional or a home visit where necessary).

We may decide that it is not in your best interests or practical for you to be registered in this way.

If you feel you fall into this category, please inform the Receptionist when you return your forms, as it may take a few more days to complete your registration. This is to enable us to make the above decisions.

Registering with Peninsula Practice

If you wish to register with Peninsula Practice, you can do so by completing a 5 minute online form which can be found here or by clicking on 'Please click here to register with us' next to this information box.

The surgery will recieve you registration information and contact you once we have checked your information.

To see your medical record, and manage your appointments and repeat prescriptions online, please complete the form on the other side of this information box, and read this information leaflet about online services and ID requirements.

If you wish to see your full medical record for dates prior to 02/02/2023, to protect the security, privacy and confidentiality of your medical record, we request two forms of VALID photo identification, for example:

Photographic UK driving licence Passport Student card
Disabled badge NHS card  

If you only have 1 form of photo identification (listed above) then you will also need to provide:

Bank statement Credit card statement P60
Mortgage statement Building society statement P45

If you have no form of photo identification then we will need to see 2 of the above.
